RESIDENCY OR SCHOOL ATTENDANCE ELIGIBILITY DOCUMENTATION
1. School Enrollment - This is the easiest way to document your players residency eligibility.
- If your player attends school within our league boundaries, please fill out the School Enrollment Form and have it signed by your school principal, vice principal or administrator. For an example of how this should be filled out, please see here
- Your player must have been enrolled and attending their school prior to October 1 of 2024. *NOTE - On the line "This student has been enrolled as of _____________" please make sure that the principal or administrator puts the date that your child enrolled at the school or started this school year, NOT the current date.
2. Residency Documentation - If your player does not attend a school within our league boundaries but you reside within our boundaries, you will need to provide three separate pieces of documentation establishing residency. This documentation is a little more complicated then the school enrollment form, so please carefully read the requirements.
- These documents must include the parent(s) or guardian(s) name, street address, city, state and zip code information, and be dated or in force between February 1, 2024 (previous year) and January 31, 2025 (current year), from one or more documents from each of the three Groups outlined below. This is to establish that you resided within our league boundaries prior to the start of our 2025 season. NO ENVELOPES - they only contain an address, and we need documents with both the address and dates of service and will not be accepted as proof of residence.
For example, you could provide:
Group 1 - a photo copy of your WA drivers license showing your Gig Harbor address
Group 2 - a copy of your November 2024 Mortgage statement with personal info redacted
Group 3 and a copy of a utility bill from November 2024
Any of the documents you provide (other than the Drivers License) must be dated between February 1, 2024 and January 31, 2025 - that means a copy of your May 2025 PSE bill will not be approved, but your November 2024 bill would be.
GROUP ONE
1. Driver’s License - Must be a Washington drivers license showing your Gig Harbor address as it’s being used to establish proof of residency. (Your Vermont drivers license that expired in 2018 will not be accepted.)
2. School records
3. Vehicle records (i.e., vehicle registration, lease agreement, etc.)
4. Employment records
5. Insurance documents
GROUP TWO
1.Welfare/child care records - child care payment receipts, child care yearly payment record for income tax records
2. Federal records - Agricultural Exemption, Social Security Summary Statement, Social Security Disability Statement, W-2, Federal Tax Return (on these documents block out your social security number and the financial figures all that we need to be able to see are name, address and date)
3. State records - Concealed Handgun License, Hunting License, Fishing License, Unemployment Records, Workers Compensation Records, Driver's License Renewal Notice (Vehicle Registration will not count, it falls under Group 1)
4. Local (Municipal) Records - Jury Summons, Property Tax Records, Traffic Citation issued by a city PD or County Sheriff, Municipal Utility Bill**
5. Support Payment Records - Alimony Payment Records, Child Support Payment Records
6. Homeowner or Tenant Records - Homeowners or Renter’s Insurance, Rental Agreement, Escrow Analysis
7. Military records - DD214, VA Records
GROUP THREE
1. Voter’s Registration
2. Utility bills (i.e., gas, electric, water/ sewer, phone, mobile phone, heating, waste disposal)
3. Financial records (i.e. loan, credit, investments, etc.)
4. Medical records
5. Internet, cable, or satellite records
*NOTE 1: Three documents from the same Group (utility bill, cable bill, and bank statement) constitute only ONE document.
**NOTE 2: Certain documents may be used in different Groups, but will count for only one Group per child. Example – If a water/sewer bill is used to satisfy Group II as a municipal record, that same bill CANNOT be used as a utility bill to satisfy Group III.
